About PT-141
"Bremelanotide (formerly PT-141) is a synthetic cyclic heptapeptide and a stabilized derivative of α-melanocyte-stimulating hormone (α-MSH), acting as a melanocortin receptor agonist. Its core mechanism involves activating MC3R and MC4R in the central nervous system, which promotes dopamine release in the medial preoptic area of the hypothalamus, thereby modulating neural pathways associated with sexual desire. Bremelanotide is structurally related to the earlier developed Melanotan-II (MT-II).
The compound received FDA approval in 2019 under the brand name Vyleesi®, for a specific indication (see below). It is administered via subcutaneous injection approximately 45 minutes before anticipated sexual activity, with a recommended dose of 1.75 mg."

"1.Improves Hypoactive Sexual Desire Disorder (HSDD) in Premenopausal Women: Two pivotal Phase III trials demonstrated that bremelanotide significantly improves sexual desire and reduces related distress compared to placebo in premenopausal women with HSDD. Its high affinity for MC4R is thought to promote dopamine release, though the exact mechanism for improving sexual desire is not fully understood.
2.Explored for Erectile Dysfunction: Early studies with an intranasal formulation of PT-141 showed statistically significant erectile responses in healthy men and sildenafil-responsive ED patients, typically occurring about 30 minutes after dosing. However, due to variable bioavailability of the intranasal route, the FDA-approved formulation is an injectable.
3.Explored for Social Cognition in Autism: A study conducted by the University of Sydney investigated the potential of PT-141 nasal spray to improve social cognition (e.g., eye gaze, emotion recognition) in autistic individuals. This research remains at an early exploratory stage."

"Approved Indication: Premenopausal women diagnosed with acquired, generalized HSDD, characterized by low sexual desire causing marked distress or interpersonal difficulty, and not due to a concurrent medical/psychiatric condition, relationship issues, or medication/substance effects. Bremelanotide is not indicated for use in postmenopausal women, men, or for improving sexual performance.
